UX Research Participant Consent Form Template
Amy Santee Career Strategy & Coaching
As a UX Researcher, you know that participant consent and privacy are of the utmost importance for research studies.
Most consent and non-disclosure forms are written by attorneys using legal jargon that doesn't make sense to your everyday person. They can be laborious to get through, so are we surprised that people just want to skip to the end and sign?
Even if your study has minimal to no risk involved, it's only right that people have a clear understanding of what they're getting involved with and how it affects them.
This simple, two-page form is written in plain language and contains the key components needed to obtain full consent from participants, including:
- Project purpose
- What's involved
- Privacy and data collection
- Data use
- A non-disclosure clause to protect proprietary product and business information
- Media release
Please note that you may still wish to have your client or employer and their legal team review the form in order to comply with business operations and policies.
After your order is complete, you will receive a link to download this file via Google Docs.
